Consent Form for Volunteers

IRB Usability Consent Form
Thank you for taking the time to participate in our usability test to improve the Texas Tech University Institutional Review Board Human Subjects website. Your participation will contribute to improving the website for faculty and students alike.
Before you begin the test, please read the following and sign at the bottom indicating that you understand and agree with these conditions.
Location: This study will take place in the English/Philosophy building, where all the materials necessary materials will be provided. Your feedback will be recorded for research purposes.
Duration: The test should take no longer than one hour. You may take a break if needed at any time.
Information Collection: During the test, you will be asked to fill out a questionnaire with your responses to tasks involving the IRB website. Please include as much information as possible; any feedback helps with the research of the site.
Voluntary Participation: If for any reason you need to leave the session, inform the facilitator and you are free to withdraw.
Videotaping: The session will be recorded on video.
Confidentiality: By singing this form, you give your consent for the Usability Research team to use your voice, likeness, comments, and video for the purpose of evaluating the IRB website. Your name or personal information will not be used.
