IRB Usability Consent Form
Thank you for taking the time to participate in our usability test to improve the Texas Tech University Institutional Review Board Human Subjects website. Your participation will contribute to improving the website for faculty and students alike.
Before you begin the test, please read the following and sign at the bottom indicating that you understand and agree with these conditions.
Location: This study will take place in the English/Philosophy building, where all the materials necessary materials will be provided. Your feedback will be recorded for research purposes.
Duration: The test should take no longer than one hour. You may take a break if needed at any time.
Information Collection: During the test, you will be asked to fill out a questionnaire with your responses to tasks involving the IRB website. Please include as much information as possible; any feedback helps with the research of the site.
Voluntary Participation: If for any reason you need to leave the session, inform the facilitator and you are free to withdraw.
Videotaping: The session will be recorded on video.
Confidentiality: By singing this form, you give your consent for the Usability Research team to use your voice, likeness, comments, and video for the purpose of evaluating the IRB website. Your name or personal information will not be used.

Recruiting Participants
Here's an update on the recruitment process:
- Until Testing sets a standard list of test times they are responsible for scheduling each participant's testing hours.
- P/S Managment is still responsible for find the the volunteers.
- After Testing sets a standard list of test times and notifies P/S Managment (which, hint hint, you could do that through the blog) P/S Managment will be responsible for scheduling participants' test times.
- Dan in P/S is in charge of recruiting in the behavioral/social sciences.
- Katrina in P/S is specifically in charge of Psychology Dept. recruitment.
- I (Bryony) am in charge of contacting Donna for participant suggestions.
- Two email addresses of participants have been sent out to Kyle and Dustin in Testing via email. If you guys need me to post those onto the site I can, but I would rather not for security reasons.
